Advance Health Care Directive FORM Until Page 6, nowhere on this form is there a place to enter the name of the person who is executing it. And the line on Page 6 is for the signature of the person. Shouldn't there be prominently, perhaps at the top of Page 1, at least the name of the person to whom the Advance Health Care Directive applies? Perhaps even the person's name and identifying info (address, SS#, age, etc.) will be necessary when this Directive becomes active.

Worst company ever! They are so racist! You won't get a chance if you have non American name. Hope their customers switch to another provider.
