About Out-patient physical therapy clinic. Story Paradigm Physical Therapy begins the rehabilitation process with a thorough evaluation for each patient. Next, an individual treatment program will be developed based upon the patients' goals, and findings from the initial evaluation. Patients will be educated about their diagnosis and how their treatment program will address their deficits and prevent any future complications. Additionally, he or she will have the anatomy/physiology, pathology, and biomechanics of the affected body part explained so that their injury is properly understood. Treatment sessions will include therapeutic and functional exercises, manual therapy, postural education, gait training, stretches, and modalities when necessary. Prior to discharge, a home exercise program will be developed to achieve and maintain maximum function.
