About We offer a full continuum of care - rehabilitation services offered along with both skilled nursing and assisted living care! Story Big Elm is comprised of an 80-bed Independent Assisted Living Center and a 50-bed Skilled Nursing Center (Dually certified) Our campus faces West A Street beside Baker’s Creek Park. The retirement center faces Glenn Avenue in the opposite direction and also adjoins the park. We are located within 2 miles of the North Carolina Research Campus.
