About We offer a boutique style private setting where one can receive their medical marijuana certification to be legal in the state of Arizona. Scottsdale Story Step 1 - Medical Records Our physician must review your medical records, including medical records from other treating physicians from the previous 12 months, your responses to conventional medications and medical therapies, and your profile on the Arizona Board of Pharmacy Controlled Substances Prescription Monitoring Program database. Step 2 - Medical Exam Our physician will conduct an in-person physical examination that is appropriate to your presenting symptoms and your medical condition, establishing and mainating your medical record in accordance with Arizona State Law. Our Physician will also explain all of the potential risks and benefits of the medical use of cannabis to you. Step 3 - Medical Card We submit all of the necessary forms to the Arizona Department of Health Services, including the Medical Cannabis Certification that specifies the patient's debilitating medical condition and states that the patient is likely to receive therapy or comfort from cannabis for the debilitating medical condition or its symptoms. You will receive your Medical Cannabis Patient ID card within 10 working days.
