POORLY I HAD A ROUTINE CHECK UP AND I HAD A PHYSICIAN DO THE CHECK UP RIGHT, SO I HAD NEEDED MY SO CALLED DOCTOR TO FILL OUT AN FORM SAYING THAT I WAS HEALTHY TO WORK WITH NO PHYSICAL CONDITION. THEY TOLD ME THAT I HAD TO PAY 70.00 FOR THE FORM TO BE FILLED OUT. NOW I HAD MY INSURANCE BILLED AND THEY STILL GO CHARGE ME MONEY TO HAVE MY FORM FILLED OUT . I NEVER AS LONG AS I BEEN WORKING IN MEDICAL FIELD THAT YOU HAVE TO PAY YOUR PRIMARY PHYSICIAN MONEY TO FILL OUT AN HEALTH FORM ESPECIALLY 70.00. I DON'T CARE IF THEY NEVER EVER SEE ME AGAIN......
