I’m not one to write reviews but feel compelled to warn anyone who is considering placing a loved one in this facility to look elsewhere. My mother was placed here after an injury. Her care was minimal at the least. She had severe edema in her legs and her doctor told the nurses to keep her legs elevated and wrapped with bandages. That worked for about a day. Every time I went to visit her, legs weren’t elevated. She would go hours before anyone checked on her. If there was a need she was told to turn on a switch so the nurse would come in. Several times she needed help to get into the bathroom and had to wait up to 25 minutes or longer - by then it would be too late. I learned that Dad would take it upon himself to help her in and out of bed or to the bathroom. This is not something a man I his 80’s with a walker should be doing. There were a couple nurses and staff who were kind and others who were rude and dismissive. I could say much more but will say that my mother improved by leaps and bounds once she left that place!!! I would NOT recommend this place to anyone!
This is a great place to work and the staff are all amazing. I have been here for six years and they management stands behind there people. The care is top notch and the patient is the priority. We strive to be the best facility in the area. If you need rehab or skilled care, I recommend checking us out.
The doctors in the day time are great, but the night crew is terrifying. They are poorly trained, they allow their mood and emotions to dictate their treatment of passionates. I would never leave a loved one their overnight, the night crew are poorly trained and will get frustrated easily, if a patient complains they will jerk them out of bed causing excruciating pain. Training to they point they yank on a newly installed pacemaker arm and tried to take blood pressure on the pacemaker arm! My mom had to sit I her own bodily waste fir 3 hours, night crew member said short staffed she do what she could when she had time. Pure
Thank you Salem Transitional Care I fell, incurred severe damage to my right ankle, foot, and leg. I spent 4 days in the hospital, sedated, yet, still in extreme pain. My 77 year old body and my 76 year old husband’s health issues made it impossible to try and make a go of it at home on our own. Knowing nothing about transitional care put us in the hands of hospital resources. The pandemic and care facility shortages put us into desperation. With much trepidation, we accepted the one and only room available in the city. God’s Grace provided SalemTransitional Care. I found myself in a lovely room surrounded by a team of people who’s sole mission was to do everything possible to make my recovery swift and painless. I had my own CNA, Nurse, and pharmacist. A cleaning crew sanitized my room every other day. The meals were above standard and right on time daily. I had a call button, that I used frequently, especially before bed time to get a snack. Physical and occupational therapy were provided seven days a week. This was the high point of my day because it gave me the encouragement to “get better”. My whining did not deter their gentle care and kind advice to support my recovery. I have never before encountered a staff on every level to be so genuinely kind and caring. My smallest request was always met with a parting, “can I help with anything else?”. Thank you all. I will never forget your invaluable care and kindness. Forever grateful, Laine Latimer
My mother was here around 4 years ago after breaking her legs. When I would go to visit I had noticed she was not herself and clearly had an altered mental state. Staff ignored my concerns for multiple days until I finally caused enough of a scene to get the right person's attention. She was sent back to the hospital and discharged to another rehab facility where she thrived and fully recovered. My step father has been in the facility for a few months post surgery. Same story, he's not himself. Unfortunately due to covid I couldn't go in person to check on his well-being. I asked multiple times to speak to a nursing director with no return call. He ended up getting sent back to the hospital when he became unresponsive. Don't allow the nice appearance of this facility deceive you. It is very poorly managed.
After a six week stay there, I have only very positive things to say about Salem Transitional Care! I arrived after a fall resulting in several broken bones and a hip replacement. I was also still very week from my condition before the fall. The staff was very caring, helpful and encouraging. The therapy staff was knowledgeable, encouraging and compassionate. The meals were good, and the facility is very clean! If there was a concern it was immediately addressed to my satisfaction. I'm very grateful for all of their help!
Feeling alarmed about this place as my mother is communicating after arriving post open heart surgery that they have no walker for her and no pads on the beds which poses potential sanitation issues. The main phones often are not attended and room phones need replaced as they have shorts in cords that either disconnect or leave a busy signal when not on the phone....the combination leaves family unable to reach loved ones at times. My mother says they don't have enough nurses as it takes sometimes hours or unreasonable amounts of time for nurses to get to rooms. She has actually called maintenance direct when a wheelchair was cutting her skin or phone wasn't working because staff was not attending to the issues. She says the positive is they leave her alone to rest and meals are provided in the room if she wants?!
It grieves me to leave this kind of review, but I feel compelled to warn people about a place like this. My mother in law was placed here and while in their care diminished to the point we were compelled to take her home and care for her ourselves. All I can say is that I saw time and time again STC staff ignore our concerns, blamed other health care agencies, or simply failed to communicate needed information in a timely way or often not at all. I would not encourage anyone under any circumstances to place someone they care about in this place. I am truly not sure if the problem is that it is poorly administered, staffed, bad policies, ineffective training, or lack of adequate equipment. Possibly a combination of them all, but the net result is still a place to avoid at all costs.
My grandma stayed here while healing from a broken hip. The staff was friendly, the facility was clean, and even the lunch was pretty good! My dad and aunt felt the communication between the staff and the extended family was excellent. It was a pleasant atmosphere to go visit Grandma in. She was well-cared for here in her transition from the hospital to a nursing home.
I have been working with the team at Salem transitional off and on for years through my work. I have always found the staff there to be attentive to patients. I have found that the coordination of actual rehab to be great. Patients come out stronger and closer to baseline than when they went in. I always know that the residents that we care for will be coming back to our community well cared for and on their way to recovery. I love working with Kori and Elena in discharge and Melissa an LPN. I'm sure everyone else is great as well, it just so happens that I communicate with them most often. The only reason this review isn't a 5 is that I don't care for the receptionist set up or the phones. I feel that someone should be reachable and that you shouldn't have to wait sometimes ten to fifteen rings for an answer. I'm hoping avamere will see that concern and change the hours of the receptionist to be more available to direct calls.
