AT-HOME Health Care is a family owned and managed agency. We began right here in the heart of the Flint Hills and have been able to serve more than 1,800 of our fellow Kansans.
We offer in-home nursing and therapy as well as home health aide services when needed. We serve patients in Lyon, Chase, Morris and Osage, Coffey and Greenwood Counties.
We are fully certified with Medicare and Medicaid as well as working with VA, BCBS and other long term care and health insurance companies.
We work with patients to help them reach their optimal level of health and to provide their physicians with an extra set of eyes in the patient’s home to prevent hospital readmission if at all possible.
