While staying at Autumn Woods, you will be cared for by physicians, registered nurses and rehabilitation personnel who specialize in physical medicine, rehabilitation and complex medical conditions.

Prior to admission, our trained professionals will meet with you and/or your family to complete a pre-admission assessment form. This information will assist our clinical team in determining if the services we provide are appropriate to meet your needs. Once admission is confirmed, we will work closely with your Physician and/or the hospital Discharge Planner to coordinate a seamless arrival.
Our on-staff VA Benefit Manager will assist you
in signing up and help you obtain all that you deserve. Our Benefit Manager will also work to help you return home if you qualify. This includes researching home benefits and spousal benefits.
