The staff seems nice but some seem a little disconnected. First meal was frozen cold and served with no utensils. The next day requested help to go to the restroom so went alone. Seems like facility is understaffed and looking for increasing profit margins. Not sure if it is a great place to stay while you're having rehab. Starting rehab so we'll see how that goes.
Went for rehab after surgery. Would not recommend it at all. It took almost 9 hours to get my meds one day. After multiple times asking. Finally got them after someone came into get them. Other times I had multiple times to get them. The food was disgusting. Only had one good therapy session in 5 days.
I cannot describe how bad this place is. Any positive review here is a lie to blind innocent people who’ll think this place is a best match for their loved one. The DON I believe her name is Natalie. Is a very horrible person. She’s rude to cna’s who stick up for themselves and for patients being poorly attended here. She’ll kick any cna out who tries to tell her she needs to help patients she even told a cna that was worried about the care here, “well what do you want me to do about it?” With such disinterest and not a single care in the world. The manager is just as awful. This place is beyond dirty. Im shocked to how it even passes health code. Most families don’t know this but most hoyer lifts here don’t even work. Your loved one will be dropped and decline fast. ive seen cockroaches lingering in the kitchen. There is definitely a cockroach and ant infestation as the other reviews claim!!!! I’ve seen many people die here due to depression and the bad treatment patients get here. PLEASE READ MY REVIEW!! What has been said is what I’ve witnessed and completely 100% true. Don’t let the positive reviews fool you!! This place does in fact needs to be reported to state. If you get ahold of your social worker please tell them how your loved one is treated here. Things will change very fast.
My Sister has had a nightmare experience here. They had insufficient and inproperly trained staff. There were ants everywhere that never got cleaned even after notifying staff. They got her medication wrong and nearly gave her a double dose of insulin, and they dropped her during a transfer and she needed x rays on her foot. That was just the first day.
In my Mom’s short time here, service appears good. The room is clean and spacious. I Live just around the corner and pray our experience stays this good.
The owners have cut back on staff workers so medications are late and services needed are hours behind. It is not the employees fault but the administrators. Read below the standard response every review gets instead of action to improve complaints.
Avalon Bountiful is a Five Star Medicare rated facility putting it in the top 10% of all facilities in the nation. They had a no deficiency survey in 2022. They staff twice the Utah state average with Four Star Staffing with over four hours of average care per patient per day. Most patients come for short-term nursing & rehabilitation and go home or a lower level of care in less than 30-days. The readmission rate to the hospital of 10% within 30-days of admission is 1/2 the Utah state average of 21%. The quality measure determined by Medicare is Five Star for Short-Stay & Long Stay patients. Avalon Bountiful has preferred relationships with The U of U, Intermountain Castel, & Lakeview Hospital to name a few. With Short-Stay Private Room Suites with private bathrooms, television, fridge, recliner, and spacious rooms. It is the facility of choice in the region. Our Rehabilitation Team has been rated best in class by the third party company Pinnacle multiple months. I would highly recommend this facility to anyone looking for skilled nursing services in the region. I would bypass other facilities to get the level of care and outcomes from Avalon Care Center Bountiful!
My husband has been in this facility for almost 8 months. The Nurses, CNA’s, Physical Therapists, etc. have been so kind and wonderful to work with. I took him home today and felt like we were leaving many friends. So grateful for Avalon Bountiful!
We have been trying to find a care facility for my family member and Avalon came highly recommended… but now I’m unsure why. Avalons admissions person Brandy Purett, she made it very clear that they were more interested in their “5 star rating” than taking care of people. Brandy brought up that 5 star rating multiple times during our conversations. It was so odd. She kept making excuses not to admit her. The two reasons she kept citing was because she had a wound from her cast rubbing and she was on a medication they didn’t like. We discussed that the medication could be could be stopped because she didn’t need it anymore, but she didn’t care. I also found it interesting a facility like this could manage a surgical wound but not a “bed sore” from a cast rubbing? Weird. It is clear Avalon is only interested in admitting the cream of the crop to help maintain that precious 5 star rating. But news flash, Avalon doesn’t have a 5 star rating, at least on Google. Just be aware that Avalon can apparently discriminate on who they admit because they are a privately owned facility. And in case you forgot, if you aren’t going to help keep up that 5 star rating… forget about it insert eye roll here.
The management nursing staff at Avalon Bountiful is exceptionally caring and involved with patient care. The rehab department provides outstanding therapy services, and usually produces great results! There is the occasional hiccup, as with any skilled nursing facility, but overall they do a wonderful job taking care of their patients!
