Description

The 21st Combat Support Hospital (CSH) was constituted as the 11th Field Hospital on 16 July 1942 and was activated 1 August 1942 at Camp White, Oregon and inactivated three years later on 21 November 1945 at Camp Myles Standish, Massachusetts.
The campaign participation credit of the unit during World War II includes Sicily with arrowhead; Naples-Foggia; Southern France with arrowhead; Rheinland; Ardennes-Alsace; and Central Europe. The unit was awarded the Meritorious Unit Commendation for service in France. The unit was reactivated 25 August 1949 in Germany. The unit was allotted to the Regular Army on 27 March 1951.
On 15 June 1952, the unit was reorganized and redesignated as the the 21st Evacuation Hospital. The unit was later inactivated on 16 August 1965. On 26 July 1967, the unit was reactivated at Fort Hood, Texas. The unit was deployed to Southwest Asia on 3 July 1991 and provided medical services in Dhahran, Saudi Arabia in support of Operation Desert Shield and Desert Storm.
The 21st Evacuation Hospital was reorganized and redesigned the 21st Combat Support Hospital on 16 December 1992 and deployed to Bosnia with Operation Joint Endeavor/Joint Guard from November 1996 to April 1997. The 21st CSH returned to Bosnia with Operation Joint Forger from September 1999 to March 2000 and deployed to Kuwait in support of Operation Enduring Freedom from January to June 2002.
The 21st CSH deployed to Iraq in March 2003 in support of Operation Iraqi Freedom 1. The unit was deployed until March 2004, when it redeployed to Fort Hood, Texas. The Hospital deployed to New Orleans, Louisiana, in October 2005 in support of hurricane Katrina relief operations. The unit was deployed until November 2005, when it redeployed to Fort Hood, Texas. The 21st CSH deployed May 2006 in support of OIF 06-08 with a unique and challenging mission of providing detainee health care. The 21st CSH conducted split-based operations, running two Multinational Forces-Iraq theater detainee interment facilities from Camp Bucca and, after the closing of Abu Ghraib, Camp Cropper. The soldiers of the 21st CSH rejoined their family and friends April 2007.
The unit deployed to Iraq in support of OIF 10-11 Operation New Dawn. The 21st CSH conducted split-based operations running three locations in Northern Iraq, Al Asad, Mosul and COB Speicher and redeployed in January 2011 from this successful deployment.
In August 0f 2014, the 21st Combat Support Hospital was once again called to duty and deployed to Kuwait in support of Operation Enduring Freedom and Operation Inherent Resolve to which they serves as the only Role 3 medical facility in the entire Central Command area of operation. The unit recently redeployed on 9 May 2015.
