Great Experience visiting a loved one. My first time here. I wasn't exactly familiar with the facility, Navigation gave me a hard time finding it. Everyone there was respectful, and Nice. They also knew the resident I was visiting. Williams come again.

I transferred my dad to Brentwood Place Four just 1 month ago and he is so happy, what a difference a month makes with people who really care! My dad has gained 6 lbs! and he is ALWAYS clean. The whole staff is so friendly & positive and REALLY care! They are definitely becoming my extended family! Thank you!!!

If I could I would give this place a 0 Star rating. My Great Aunt resides here, her and her roommate are constantly neglected. I gave her roommate a blanket because the staff refused to bring her an extra, so I brought one for her and wrote her name on it. It has disappeared and every time I ask for it "It is washing." Their room constantly smells like feces. The people "caring" for these older adults are not doing their job in providing a livable space. The smell of feces is not only unpleasant but cause sicknesses. They also refuse to give us an invoice that outlines the cost of the facility because what they are charging our bank account is not a constant rate. The account manager will contiguously harass us and threaten to kick my great aunt out if the payment does not go through but they over charge the account. Even though the charged amount should be the same every month. Currently looking into calling APS and moving my Aunt because this place is not a great health care provider.

Brentwood Place Four is the place to be. I work here for 4 years and finding a good facility with great staff that serve our patients well can be difficult at times. This building and their staff are the best! If you have a loved one needing rehab or long term care, please visit us.

My son is at Brentwood Place Four and has been there for couple of months. The staff there are very professional and6 nice they take great care of my son and all of their residents. I live the administrator there Mr Chudy. He listens and is always happy to help whenever you call on him. I will always recommend anyone to Brentwood Place Four as long as he is there.

I had to come on here and leave a thank you to Brentwood Place Four for hiring Ms Jackie Tyler. I,very been in this facility several times over the last few years visiting with a few women from my church and I've never observed so many residents happy and staff cheerful and helpful. I asked a few residents what was happening and if there was a change in staff/ management. I was so excited to see the satisfaction on their face when they started talking about the new ADON, Jackie Tyler. I have yet to meet her but I pray she stay and that who ever hired her to continue hiring that caliper of employees. So congratulations to the management team. I have always liked the facility and it is a refreshing feeling to know God still answer feeling prayers.... Beth Crosby.

I visited my sister today and everything was great The PCT was very attentive to my sister! Staff was engage and making rounds The Administration’r and DON were both very attentive to concerns and address each one immediately I feel comfortable with my sister Rosie at Brent Wood 4 Thank you Administrative and the DON, you both are very attentive to the patient’ s need! My dad has been here for a few months. I visit him when I can and from what I see and hear from my dad, the food is COLD, linen is not changed daily, they don't give him a proper shower in hot running water, a spung bath is not proper cleaning to anyone body. He doesn't get his physical therapy daily either and just lay in the bed which will eventually cause BED SORES. I see the Director Mr. Chudy took my grandmother phone and posted a review as if he was her to make the facility appear to be on top of things but they are not. My dad will be removed soon.

I always feel welcome. I like the kiosk that automatically checks you in for your visit. My brother looks great folding his sheets and putting them away and making up his bedcause he wants to which is great for a double amputee. I believe it gives him self worth.

Brentwood Place Four is an excellent place to work. Staff displays caring and dependability. Truly provides a family atmosphere for residents, families and staff.
