We now accept some insurance for immunizations. Make sure your carrier will pay for the immunizations before arriving.
We DO accept Credit/Debit Cards for a minimal fee.
No appointments necessary as long as you come in between the hours and days allowed for each service listed:
Birth & Death Certificates are printed Monday - Friday 8:30AM-4:00PM
Child Immunizations: (Birth-18yrs)
Monday 1:00PM -3:00PM
Tuesday and Wednesday 8:30AM-11:00AM & 1:00PM-3:00PM
*Please call ahead if your child is due for immunizations.
*Bring a copy of your childs immunization record when you come.
*If your child is 17 yrs or younger they must be accompained by a parent or guardian to receive immunizations.
* PLEASE contact the office for scheduling and eligibility requirements.
Lab Draws:
Mon,Tues,Wed & Thurs: Lab Hours from 8:30 to NOON only
ALL LABS MUST HAVE A DOCTORS ORDER (Fasting may be required for certain test, ask your Dr. prior to coming in)
Adult Immunization Days & Other Nursing Services:
Monday, Tuesday, Wednesday & Thursday 8:30AM-11:00AM & 1:00PM-3:00PM
* PLEASE contact the office for vaccine availablity.
Other Services:
i.e. Pregnancy tests ($3.00), TB testing ($8.00), STD testing, Free Condoms etc...
NO NURSING SERVICES ON FRIDAY
