Typically I went to see my dad in the afternoon. Saturday July 22 I went in at 10:00 a.m. I found my dad without any clothing and flat on his back with no water or oxygen within reach. He was dying from congestive heart failure. Everybody knows you do not lay a person flat that has congestive heart failure. It was a complete nightmare and I removed my dad 2 days later. A pathways hospice nurse Triva was incredibly unprofessional and talked me out of removing him immediately saying he "has hours to live" which was not true. I do have photos of this but obviously I will not share them with the public. This place is definitely a place not to take your loved one.

I talked to the supervisor, the nursing staff, about my father having none of his antibiotics, and cleaning up his own blood off the bathroom floor, as the call light was on, nobody showed up. Was told doctor would be there Monday to check his surgical wound. Never happened. Called ambulance and left.

Although most staff seems friendly, they didn't seem interested in admitting my LO. They didn't seem concerned with her needs and requirements. Once I picked up my LO to bring her home, her cath bag was so full it was about to explode, she was left to sit in feces and they ripped her toenail off and nobody knew how. It was a heartbreaking experience that I refuse to ever let happen again. I do not recommend this place. I did give 2 stars because a couple of the nurses seemed to make a heartfelt impact on my LO.

The staff at Colombine were wonderful. I enjoyed my stay at Centre Ave health and rehab. Everyone was thoughtful and helpful. Please give my regards to everyone.

My mother had a fantastic experience. The staff was so kind, attentive, and friendly. She just could not say enough about the care she got from every person. The OT, PT and doctor that attended to her listened closely and worked diligently to get her healthy. The nurses and aids were so kind and seemed to genuinely enjoy their work. And big bonus was the food was "sooo good." My mom inisisted that I sit right down and write a review.

Our experience at this re-hab facility was terrible, my wife asked if they would feed her as others were being feed as she has severe arthritis in her right hand and can not use her left hand do to a stroke, the nutritionist said they would accommodate her,however because she was attempting to use there utensils they did not see the need to feed her per case manager asked a second time and they still did not feed her after 3 weeks and a loss of 9 pounds i pulled her out of this re-hab facility finale weight 103

For the most part, the staff is great. However, the nurses are understaffed and overworked. They are great; they just can't handle everything. So visit your loved one often and ask a lot of questions. The people at the front desk are very friendly and greet my step mother by name. The PT staff made significant progress with my dad when The Centre at Rock Creek hardly made any progress in the 2 1/2 months that he was there. All in all, this is one of the better run Columbine facilities.

They rarely have the same staff working with the same residents, this leads to isolation and miscommunication regularly. Have to call back at the beginning of shift changes to ensure continuity of care

Centre told us that our loved one was "not progressing" and that we had 6 days to find a skilled Nursing Home for her. We relocated her to The Green Home at Marisol in Loveland. The FIRST day that she was there, she was up and walking. 2 weeks later, we are looking at her going home to assisted living...

Great company to work for but when I was working there supervisor was very upset because I had to do my two weeks notice, cause of my daughter so yes I did my two week's and extra day for her. So later on I Tried to apply back for Dietary Aide position they kept denying my application cause supposedly I didn't do my two week's when I did it was all angre from my old supervisor plus she always kept calling me to.please come back blah, blah , blah but I had told her I couldn't because I still didn't have a decent reliable babysitter so she made false information about me she knew how bad I wanted to come back to work and she knew my goals we're to become a C.N.A and work for this company and she blew my chances away for no reason for false information.
