I recently had an elderly friend that needed rehabilitation care after a bad fall. I was lucky to be able to work with Josephine in Admissions who made the transfer from the hospital to their facility seamless. She was extremely helpful in guiding us through the process as we have never had to navigate something like this before. The Nurses and Physical Therapy staff were on point. All his needs were met and they helped him literally get back on his feet. He is set to be discharged this week and we couldn't be happier. Thank you again for all that you did! The entire team was easy to work with and efficient.

I went to Concord Healthcare & Rehabilitation to visit a family member. This facility was clean, no strong orders, and the head of maintenance and his staff were right there ensuring clients rooms were clean, garbage free, bathrooms cleaned and stocked with toiletries. The front receptionist was friendly, accommodating, and explained the check in process. She couldn't be more helpful. I also really liked that there was a check in process which was a contributing factor to the safety and security of their clients. My family member and other clients I saw during my visit were well taken care of with clean with neat clothes. My family member was benefitting from his schedule of PT and direction of these facilitaters. This all takes a great effort of teamwork from all aspects of this facility from the administration, nursing, client assistants, and mainteance. I would highly recommend this facility.

I have nothing but good things to say about Concord. They always went above and beyond to make my great grandma feel comfortable and cared for. The staff was very kind and patient to both her and our family, especially when keeping us updated and informed. I am eternally grateful for the love that was consistently given by everyone at Concord and could not recommend them enough!

I am so happy with the care my great grandmother received at Concord. She was only there for a few months, but even still, she grew so close to all of the nurses, doctors, and staff members. While she was in hospice, Concord was there for not only my grandma, but for me and my family. Everyone at Concord showered my family and I with prayers and love during a most difficult time. Thank you Concord for taking care of my Grandma. I will never forget all you did for her up until her last days.

Concord Health is an amazing facility! The staff is friendly and knowledgeable, and the facility is clean and well-maintained. I had a great experience during my visit. Highly recommend!

My husband's grandmother was a patient here until she passed away in November. Very grateful to the nursing and therapy staff for taking excellent care of her. Thankful for unit managers Lori & Michele for addressing any issue or concern I had about anything during her entire stay. Grandpa wanted to give a shout out to nurses Charles and Jacob for always being extra kind and helpful while taking care of her and another thank you to Lori & Josephine for being by our side as she transitioned to heaven. I can't thank you all enough for everything. Concord is truly filled with angels on Earth.

My family and I will forever be grateful to our Concord family for the outstanding care provided to my grandmother from day 1. She admitted to us a few months ago while she was declining at home. The entire team went above and beyond to ensure her welcoming and transition into the facility. She felt extremely reassured by the nurses, aides, therapist, housekeeping, and the list goes on... A few that really stand out are from nursing: Vernique, Nicole, Rory, Debbie, Kesha, Wanda Devorah, Donna Cheryl. And many others truly went above and beyond for her. She developed a very close relationship with all of these amazing individuals. Also Judy, Patricia, and Emma went out of there way to constantly check on her and spent a great amount of time with her. Our amazing administrator Jay ensured all her needs were met as he does with each patients that comes trough our door. Cathy our DON and Lisa our ADON are amazing at what they do. During her last days as she transitioned, my family and I were humbled by the outpouring love and support by the staff. We will forever be thankful to EVERYONE at Concord. There are many more names to mentioned but please know that everyone was amazing. May God Bless Concord and all the residents and staff members affiliated with us.

My father, who is 100 years old, was a patient at Concord. When he got to Concord from the hospital he was not doing so well and in a wheelchair. Presently, he has moved on. Dad is walking and enjoying his life. The staff is tops!!! Each one deserves a shout out, but the list is too long. So a HUGE THANK YOU to all the staff for your exemplary work. My father recently arrived here after a week in the hospital. I am extremely grateful to Josephine for her assistance with the move from the hospital. She made our lives a little easier, while we are working along this transition. She has made herself available to my brothers and myself for any questions or requests we have had. She is Great ! We were immediately greeted at the front door and taken to his room. Within the first 15 minutes of arriving we were greeted by different nurses, administrators, and therapist. They really did a good job of putting us at ease, but more importantly they made my father comfortable.
