Who is Eligible?
A patient can receive services through his/her insurance, Medicare, or MEDICAID COVERAGE when he/she meets the following conditions:
Patient is home bound (has difficulty and requires assistance in leaving their home)
Service is prescribed by the patient’s physician
Nursing or Therapy service is part of the plan of care
