Freedom Home Health and Hospice Care Services Inc. provides palliative care in your place of residence (home or facility). Palliative Care is defined by the World Health Organization as an approach to care which improves quality of life of patients and their families facing life-threatening illness through prevention, assessment, and treatment of pain and other physical, psychological, and spiritual problems. This approach to care improves quality of life for patients and families facing life-threatening illness through prevention, assessment, and treatment of pain and other physical, psychological, and spiritual issues. We provide short term inpatient services, mobilization and coordination of ancillary services and bereavement support. We use an interdisciplinary group of physicians, nurses, social workers, homemakers, hospice aids, volunteers, spiritual counselors, bereavement counselors and others to meet your specific needs. We offer guidance to help you determine what types of care are most appropriate for your needs and the needs of your loved ones. We help identify what services will be included as how the billing process can be simplified. We are available for you, a loved one, a friend or family member.

Home Health Criteria:
To qualify for Medicare home health services a patient must:
Be homebound and a doctor must certify that you are homebound
Be under the care of a physician who is a doctor of medicine, a doctor of osteopathy, or a doctor of podiatric medicine
A doctor must certify that you have a specific skilled need
We will work with your doctor and you to create a Plan of Care (POC) that includes:
