Patient Navigator Program - Do you need help understanding your chronic disease? Communicating with your doctors? Understanding your medical bills? Making sure you have everything you need to be healthy? We coordinate care and work with you to guide you through the healthcare system and to community resources to help you get the care you need when you need it.
The A.C.H.E.S. & P.A.I.N.S. healthcare voucher program -- financial assistance for uninsured or low-income children and adults for acute medical, dental (children only) or mental healthcare issues. Prescription assistance and gas cards for medical trips also available.
