Description
The Hanover Family Health Team provides primary health care, including chronic disease management, health promotion and disease prevention, to a population of rostered patients in the Town of Hanover and surrounding area. The FHT employs an interdisciplinary team of health professionals who work collaboratively in a patient centred model of care.
