Heartland Health Centers (HHC) is nonprofit community health center serving Chicago's north side.
The mission of Heartland Health Centers is to improve the well-being of the communities we serve by providing accessible, high-quality healthcare. We are an affiliate of Heartland Alliance for Human Needs & Human Rights, an agency that has helped people living in poverty or danger to improve their lives and realize their human rights since 1888. We provide a medical home on the north side of Chicago where patients may receive medical care, prenatal care, pediatric care, mental health care, oral health care, and prescription discounts regardless of their ability to pay.
• Last year, HIHC provided nearly 60,000 visits to more than 14,000 patients.
• The majority of HIHC's Board of Directors is comprised of active HIHC patients.
• Every HIHC site uses an integrated electronic health records system.
• Through in-person and telephone-based translation, services are available in over 35 languages.
• All HIHC family practice physicians hold faculty positions within the Department of Family and Community Medicine at Northwestern University's Feinberg School of Medicine.
