We are a Family Practice located in Honeoye Falls, NY. We have office hours on Monday - Saturday and offer routine & urgent care appointments during our extended evening hours on Monday and Wednesday from 5:30-8:30pm and on Saturdays 8:00 am - Noon. Dr. Frese & Dr. Cromwell are accepting new patients.
Over the years, we have established excellent working relationships with a number of area hospitals and community based behavioral Healthcare specialist. Referrals, as well as coordination of behavioral Healthcare services are created by our providers to these specialist and have provided help to individuals and families alike, making a lasting difference in their lives.
We are affiliated with AHP & NCQA recognized as 2011 PCMH Level 3 and Excellence in Diabetes care.
