HTM Consultants assigns a nurse to each patient to assist in maintaining the best quality of life possible. This means that one person is actively involved in overseeing all aspects of your care to ensure that you receive complete and competent care. Understanding and addressing the problems of each patient is our specialty. We help evaluate your home and health conditions, provider referral assistance, creating a care plan based on your medical conditions and situation by improving your quality of life. We assist in choosing a facility and level of care if a patient chooses to move out of the home. We help you get the care you deserve by assisting with appointment scheduling, outreach to community resources, medication management and promote independence through patient advocacy. We bridge the gap between your doctor appointments and decrease ER/hospital visits.
Our mission is to provide the necessary advocacy support and financial needs to Veterans, Geriatrics and all others in need with the care they deserve. We know how difficult it is to navigate through the healthcare system and we want to help make this process easy and efficient for everyone. We promote caregivers and family members to be involved in their care but let us do the "heavy lifting" so they may enjoy their time together. We know how to locate the resources necessary for these individuals and identify their needs to get them met quickly. Helping and healing others is what we do.........mind, body and spirit.
