With quality a primary goal, we leverage a technology driven in-home health risk assessment to identify gaps in care while providing relevant and accurate data providers need to fill those gaps.
1) Health Risk Assessment (HRA):
The in-home health risk assessment is the cornerstone of our company’s service. The HRA accurately identifies and anticipates risks so care plans can be tailored to deliver care when it is needed most, and in the most cost-effective and efficient manner.
It is conducted by a board-certified nurse practitioner. The results are then given to the health-plan provider for review and dissemination to the assigned primary care provider (PCP) at the plan’s discretion. During the course of the visit, the member can expect the following:
• A comprehensive medical history review
• A physical examination
• A medication review and reconciliation
• Psychosocial, financial, and caregiver needs assessments
• A diagnosis(es) capture
• Education regarding health promotion and disease prevention
2) Member Targeting & Analytics:
We use your member data (historical claims data, prescriptions, labs, socioeconomic, demographic, etc) to decide who should have an HRA, when they should have it, and what else they may need. We then use it to inform our clinicians before they visit the member, providing a base layer of knowledge the member may or may not recall. Data collection is what allows us to maximize our clients’ resources to deliver the right care to the right member.
3) Risk Adjustment Focused Coding:
Once an HRA has been completed for a member, Ingenios Health will accurately code each form and thoroughly identify all ICD-9(10) codes and their matching HCC codes. This allows your health plan to receive the optimal reimbursement from CMS. Our coding services are specifically focused on CMS risk-adjustment to ensure the most value for our clients.
