In your home a homecare professional team provides individual assessment, care, and teaching for both patient and family members. In addition to your home health nurse, your care team includes: your physician, physical, occupational and speech therapist, home health aides and case management. Additionally, a nurse is always available to you 24 hours a day, seven days a week.
