Community Health Network Area 12: Amesbury, Boxford, Georgetown, Groveland, Haverhill, Merrimac, Newbury, Newburyport, Rowley, Salisbury, West Newbury
Health Partnership of the Lower Merrimack Valley and Seacoast is one of twenty-seven Community Health Network Areas (CHNA) local collaboratives across the Commonwealth of Massachusetts formed in 1992 by the Massachusetts Department of Public Health. Formally known as the CHNA 12 of Haverhill, the health network rebranded in January 2014 to the Health Partnership (HP) of the Lower Merrimack Valley and Seacoast. Through its collaboration with health and human service providers, community based organizations, faith-based groups, coalitions, residents, businesses, community leaders, and municipal governments, the HP seeks to improve the health outcomes in the communities we serve and increase communication among service providers, consumers, and residents. The Health Partnership includes the following cities and town: Amesbury, Boxford, Georgetown, Groveland, Haverhill, Merrimac, Newbury Newburyport, Rowley, Salisbury, and West Newbury.
The Health Partnerships mission is to improve the health status of the communities.
