New Hampshire Health & Equity Partnership is a collaborative effort to advance health equity for NH’s racial, ethnic, and linguistic minority communities.
The New Hampshire Health & Equity Partnership (NH H&EP) is a public-private collaborative of community based organizations, public health agencies, philanthropic organizations, advocates, consumers and others concerned with health equity. We are working together on innovative solutions that remove barriers and expand pathways to health, thereby improving the opportunity for good health for everyone and a prosperous future for our state.
The Partnership is guided in its work by the Plan to Address Health Disparities and Promote Health Equity in New Hampshire. The goal of the plan is to define statewide priorities and prepare recommendations to advance health equity for NH’s racial, ethnic, and linguistic minority communities.
Our Vision is “Everyone in New Hampshire has a fair opportunity to live a long, healthy life”.
The Plan’s framework of strategic priority areas includes:
• Access to Health Care
• Addressing Environments Where We Live, Learn, Work and Play to Improve Health
• Awareness and Promotion of Health Equity
• Improving Data Collection and Use to Identify Issues.
The Plan to Address Health Disparities and Promote Health Equity in New Hampshire considers health equity within the broad framework of social determinants such as education, housing, employment, environment, and other factors that influence health status and access to quality health care for individuals and communities. By addressing health disparities at their source we will improve health and health care for minority communities and for the general population statewide.
