We are a primary care organization located in Vermilion Bay Servicing the Municipality of Machin.
The Points North Family Health Team is patient centred. Patients will have access to a number of primary health care services from a variety of team members, including a family doctor. The team approach means service is relatively easy to access on a timely basis. Patients will be followed by a doctor or nurse practitioner, but depending on their clinical needs, they may be referred to other health care professionals within the Family Health Team. Patients may also self-refer to a Social Service worker.
The team approach often allows for more time and discussion of health concerns than might be possible in a more traditional medical office setting. Also, patients will have access to additional services such as hypertension management programs, Diabetes and smoking cessation counseling.
Diabetes Care, Chronic Pain Management, Mental Health, Health Advocacy, Hypertension Management, Cancer Screening, and all services related to primary care.
