*Give Help- Suzanne Jacobson Memorial Fund
Our vision is to achieve philanthropic success through donor cultivation, financial stewardship and a collaorative relationship with Prairie Lakes Helathcare System.

In our lifetime we may have the opportunity to make a profound improvement in the quality of our community for future generations. Individuals who wish to make a significant gift to the PLHF of $10,000 in cash or securities over five years will be a member of the Friends of the Foundation Giving Club. Installments may be made over the five year period.
100% of your gift remains in our community to help more patients, expand facilities, acquire cutting-edge equipment and atract top medical professionals.
