Sommerville Family Practice provides comprehensive medical care for the entire family. We utilize the patient-centered medical home model and strive to provide you with the best, personalized medical care available. The practice currently provides out-patient care for ages 2 years and older. If you are a patient of our practice, and you call before 10:00 AM, we guarantee an appointment that day. Sommerville Family Practice sets high standards for excellence. Major innovations from the American Academy of Family Practice's Practice 2010 initiative make Sommerville unique in ways that improve safety and convenience for medical care. We were chosen as one of 36 demonstration practices helping define the future of Family Medicine in the TransforMED National Demonstration Project. The practice uses electronic medial records and at no charge, once you establish a Patient Portal account you may contact us via e-mail, review lab results, and schedule appointments.
