Southern Choices, LLC Community Supports Services offers specialized services in several regions and is comprised of the following programs:
1) Community Residential Services
-Developmentally Disabled
-Dual Diagnosed
2) Community Living Services
Personal Care Home
-Provided for developmentally disabled.
-Trained staff provides watchful care and training in daily living skills, money management and responsible community behavior.
Semi-Independent
Living Arrangements
-Designed for adults who have the necessary basic skills to live without 24-hour supervision and live with families.
-Outcome is for these individuals to live as independently as possible.
Supportive Living
-Provides assistance to individuals who need help with placement and/or the supports necessary to maintain a stable living situation.
-Assistance with securing an appropriate placement based on skills necessary to maintain placement.
-Services include: help with budgeting, housekeeping skills, shopping, health maintenance & nutritional education, transportation to medical appointments, assistance with obtaining and managing prescribed medications, and accessing community resources.
Our mission is to provide quality care and services to individuals with developmental disabilities that recognizes each individual's dignity empowering them to gain a more independent life style and self-determination to maximize one's own potential.
At Southern Choices, LLC we provide trained, professional, caring staff that will provide your health care needs in a confidential setting. You, as an individual, are encouraged to be involved in your treatment planning process as well as your family or significant others.
