Take Care’s professional, experienced Registered Nurse Geriatric Care Managers oversee and manage one’s total health care, including crisis prevention and planning. Take Care understands the need for a proactive approach. In recognizing a fragmented health care system, Take Care Advisor was formed to offer registered nurse geriatric care managers who guide older adults in managing all aspects of one’s health care. An integral part of this program is conducting a professional geriatric care assessment, a health insurance review, and formulating a plan for long-term care.
