Description
Each person and/or family seeking assistance requires a referral from a church, school, or social agency. The referral should include the names of all household members, Social Security numbers for all persons 18+, and the specific needs (food, clothing, bed, washer, etc.). Photo ID is required to prove residency in the supported areas.
