Requirements to Receive Home Care:
1. Must be homebound
2. Be under the care of a Physician
3. Must be recovering from an illness, hospital stay or injury, or demonstrate a scope for improved health & wellness
If the above criteria is met, we offer when medically necessary:
Skilled Nursing (RN)
Physical Therapy (RPT)
Occupational Therapy (OTR)
Speech Therapist (SPT)
Medical Social Worker (MSW)
Home Health Aide (HHA)
Registered Dietician (RD)
Diabetic Counselor (RN)
We can send a visiting physician pending your primary care physician's order to do so.
