Skilled nursing services provide care to those who have been discharged from the hospital, but require comprehensive care and rehabilitation services. Our caring nursing and rehabilitative team members provide a bridge between the hospital and your home.

Long-Term Care
Westchester Gardens Health & Rehabilitation provides long-term care when the individual’s needs exceed a family’s ability to provide care at home. Here you will find experienced nursing team members who are dedicated to provide high-quality care. Professional team members, including registered and licensed practical nurses and certified nursing assistants, are on duty 24 hours a day. Residents are also seen regularly by their personal physician.
