Five Oaks Manor is a short-term rehabilitation and long-term care facility located in Concord, NC. We provide physical, occupational, and speech therapy.
Five Oaks Manor is pleased to present a newly renovated skilled nursing facility. New floors, paint, trim and lighting was just the beginning. Renovations also included new remote controlled beds and furniture in all of our long term, short term and assisted living rooms. Our facility boasts some of the largest resident rooms in the county. A variety of cozy spaces within the facility were created for residents to meet with their families and friends with a more home-like feeling. Five Oaks Manor has added Wi-Fi so that our tech savvy residents can bring their personal computer or laptop and stay connected to their loved ones through E-mail.
One thing that has not changed is the high level of services we provide to our residents, including skilled nursing care, restorative nursing, a variety of activities, in-house rehabilitation team and employing staff that value and respect the diversity of all of the residents within the facility.
For over 35 years, Five Oaks Manor has been part of the community with continuous services from our medical director, Doctor George Hall. Along with being the bridge between hospital and home, we offer a variety of services to promote motivation, self-expression, self-reliance, and independence. Our team of physical, speech and occupational therapists along with our skilled nursing staff will strive to make your stay comfortable and beneficial. We have a variety of activities and social functions to meet individuals mental and physical capabilities. Our staff is dedicating to meeting the social, emotional, and spiritual needs of our residents.
