If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:

•Driver's License or State ID
•Referral (If Required by Insurer)
•Insurance Information
•Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
•A List of Current Medications and Any Known Allergies

Dr. Greg Carr is a wonderful and very kind doctor! He has taken very good care of me, from my injury, through total knee replacement and several scopes throughout the years. I highly recommend him... his staff reflect his kindness and professionalism!
SOS is a friendly family atmosphere. You can feel the warm welcoming and smiling faces. Dr. Carr is a Christian physician who cares and take pride in his work.
Absolutely awful. Waited 2 hours to be seen for 5 minutes. Dr. Carr was incredibly patronizing and unhelpful. I left in tears. He continuously tried to discount an official diagnose I have, while asking me to do things I told him would injure me. He treated me like a child. AVOID!!
Great experience overall! It didn't take too long to be brought to a room and the doctor gave me a great explanation of the diagnosis and his treatment. The staff was so nice and professional too! You can tell Dr. Carr really cares about his patients.
The Dr. And nurses were great the got me in and out in no time. The facility was wonderful. The surgery went great, l only wish l had not waited so long.
As I said no I've never experienced anything here at this office I trust my doctor who I had for eight years and would not steer me wrong and he gave them a high recommendation which means to me that there were five stars
