TELEHEALTH MONITORING:
* Encourages clients to take health related measurements in their home. With the current equipment
chosen by BAYADA, clients can measure blood pressure, heart rate, weight, blood glucose and/or oxygen saturation. Results of
these measurements are transmitted via telephone lines to BAYADA office staff Results are then evaluated according to
physician orders and/or agency policies.
* GOALS:
1. Promotion of client self management skills related to his/her disease processes.
2. Early detection of signs and symptoms of disease exacerbation's and complications.
3. Reduction of emergent care needs and re hospitalization.
4. Improvement in client outcomes.
5. Improvement in client satisfaction
ZONE TOOLS: Decrease hospial readmissions, keep clients safe at home.
* Types: Respiratory, cardiovascular, fall risk, Wounds, seizure, glucose management
*The GREEN zone reflects what your client “looks” like when he/she is well. This tool provides you with a quick reference to the client’s baseline
*The YELLOW zone reflects changes in the client’s behavior or assessment findings. There will be PRN interventions that can be initiated to support the client.
* RED Zone EMS must be activated at this point. The interventions that have been implemented for the client have been unsuccessful. The client requires advanced care that cannot be provided in the home care setting and it is no longer safe for the client to remain in the home
