Cherokee Nation Home Health Services is made up of three service teams. Cherokee Nation Home Health provides home health services to those in need of short term medical care in their homes. Cherokee Nation Home Health Outreach provides ADvantage Waiver and State Plan Personal Care Services. Hospice of the Cherokee provides hospice services to those diagnosed with a terminal illness.
