De Baca Family Practice is a full service federally qualified health center serving the families and individuals of De Baca and surrounding counties.
De Baca Family Practice Clinic provides a full array of medical, dental, and integrated behavioral health services, as well as lab and x-ray services. At the Main clinic in Fort Sumner we provide all of the above mentioned services, at Fort Sumner School-based clinic we provide the same services in a more limited format, at Guadalupe Family Dental in Santa Rosa we provide dental services.
It is our mission to to provide comprehensive primary and preventive health care in a medical home environment to people of all ages, backgrounds and life circumstances, regardless of their ability to pay.
