We teach, provide services and support individuals living with diabetes and their families so that they can attain a healthy lifestyle.
We promote awareness about factors contributing to diabetes to minimize the impact of this disease on the people of our community.
Education - one on one sessions in our office with a diabetes educator & group classes or presentations for employers and their employees. Call for pricing, special Medicare rate and a sliding fee scale is available.
Advocacy - does glucose testing events throughout the year as well as sits on committees dedicated to promoting healthy lifestyles throughout our community.
Labs - we are certified to do blood testing by the state of Indiana. We have Hemoglobin A1c tests, Lipid panels, and fasting glucose tests available by appointment. These tests are already at the lowest possible price when you use a referral and therefore we do not accept the sliding fee scale for them.
Support Services - assesses the needs of a diabetic to give them proper referrals, enroll them into private and government programs along with getting them assistance from the pharmaceutical assistance programs for free medication and supplies.
Financial Assistance - must meet financial eligibility requirements and live in St. Joseph County for this program. Defrays the cost of medication and supplies for un or underinsured diabetics in our community keeping them healthy and out of our ERs & hospitals, lowering unnecessary medical costs.
