On Tuesdays, these offices will be open from 11:00am - 7:30pm.

Preventive Services
• “Well” assessment, basic physicals, and immunizations, preventive screenings
Illness Assessment, Treatment, and Disease Management
• Simple medical procedures, In-house lab services, prescription of medication, chronic disease management, Substance Abuse and Dependence assessment and care
Women’s Health Services
• Family planning services, first trimester prenatal care for low-risk patients and prenatal care coordination with OB/GYN
Specialty Care
• Identification of specialty care needs and referral to specialists, care coordination and follow-up, assessment and ordering of services for home care
Mental Health & Wellness Services
• Behavioral health assessment and brief focused intervention therapy with licensed mental health professionals
• Assessment and integration of alternative wellness options and education classes
• Preventive health coaching, goal setting, and anticipatory guidance
Care Coordination & Patient Communication
• Telephonic and in-person follow-up regarding hospital discharge, emergency room visits, procedures, acute care follow-ups, and specialist visits
• Medication reconciliation, self-care planning, and treatment plan development
• Assess and provide linkages to community resources when indicated
