Evergreen Health is a not-for-profit organization that fosters healthy communities by providing medical, supportive and behavioral services throughout WNY.
Evergreen Health provides medical, pharmacy, housing, mental health, nutrition, transportation and syringe exchange services as well as health education, health promotion and disease-specific prevention programs to over 13,000 individuals from within our four facilities located in Buffalo and Jamestown, NY.
Over the last 30 years, Evergreen Health has developed a wide range of life-enhancing services to support the ability of our clients to lead healthy, secure and satisfying lives. We work with underserved and/or stigmatized individuals dealing with multiple chronic diseases such as HIV/AIDS, diabetes and heart disease. We also provide a variety of services to assist those with mental health challenges and substance use issues, and offer programs created specifically to assist sexual minorities - many of whom are inadequately treated by mainstream providers.
Evergreen Health is recognized by the Human Rights Campaign Foundation as a "Leader in LGBT Healthcare Equality," and our Medical Group is a designated Level 3 Patient Centered Medical Home - the highest achievable recognition from the National Committee for Quality Assurance Physician Practice Connections.
