Forest Ridge Senior Living is a skilled nursing and rehabilitation facility located in Woodland Park, CO. We offer both transitional and long-term care, depending on the individual needs of the patient.

Wonderful, beautiful facility and caring staff. HORRIBLE administration!! Over the course of 3 months last year 2021, more than 15 CNAs quit. At anywhere between $3K and $8K a month per resident, administration couldn't pay the CNAs $2 more an hour to stay and then realized that agencies were MUCH more expensive, and so decided they didn't want to use agencies either, leaving the remaining staff seriously over-worked and under paid. The current administrator told a patient who had been sent out by the dayshift nurse a former ER nurse - sending her out saved her life that they valued their five-star rating and so next time she needed to be sent out, she needed to clear it with administration first. This facility is in violation of State Law in their "memory care" lock-down unit, because they are not adequately staffed to handle the sometimes severe and DANGEROUS personality changes in residents. When a resident on this unit w/o mental decline felt violated by another resident, the nurse on duty followed STATE PROTOCOL as a mandatory reporter and notified APS. The DoN at the time AND the administrator chastised her for this, and the DoN told her that she needed to have notified the DoN first NOT true, and the admin wanted to know why the RN felt that this case qualified as reportable to APS. Well DUH! The resident was CRYING AND FELT UNSAFE IN THE FACILITY!! The administrator is a JOKE. She was the activities director before taking A CLASS not a course on becoming a nursing home administrator, passed, and because of her personal non-business relationship with the owner, she was suddenly in charge of a facility full of health care personnel. She claimed to be a CNA and yet, when she came to help out with vitals on Mother's Day last year, a CNA on the unit had to show her how to use a pulse ox, and a DIETARY AIDE had to show her how to count respirations!! She wrote up a tenured nurse for not wearing her mask in the hallway because the nurse had to pull it down to use her inhaler. The admin told her that next time she needed to use her inhaler, she needed to do so in the "designated drinking areas" rather than in the "patient areas". Like the nurse can plan when she needs an inhaler!! Again, beautiful facility, all private rooms w/ private bathrooms, and great floor nurses and CNAs, but TERRIBLE, HORRIBLE, AWFUL administration, at least, the DoN at the time, the owner, and the current administrator. I didn't really have much interaction or any issues with other members of administration.
I recently returned home from an 8 day stay after a total knee replacement. I can't say enough good about the care I received. The facility is clean and inviting. The staff is Fantastic. I rehabbed here after my first knee was replaced and came back after the second. Did I mention the staff is Fantastic? We are lucky to have a facility like this in Woodland Park. Thank you to all of you!
I have the privilege to work at Forest ridge. I am thankful to be part of such a great team of people caring for such a great house of residents! I highly recommend Forest Ridge to anyone looking for a skilled nursing or long term care facility for their loved one. In my 30+ years of nursing and caring for our elderly this is one of the best facilities out there!
The facility was a great place for our severly disabled mother at first but that changed after a the new administrator came on board. Let me be very clear that there was zero quality assurance executed by this facility's head nurse. Our mother developed reoccurring pneumonia, RSV, and influenza. Also left unattended in her room without oxygen during her struggle with pneumonia. We removed her from this facility after she lost 20 plus lbs after they closed the doors due to covid and failed to communicate with us until sepsis developed from the pneumonia. We could not believe how many things went wrong with this facility in such a short period of time. If you must place a loved one in this facility be sure to follow closely especially if your loved one lacks the ability to articulate.
We waited with baited breath for the opening of Forest Ridge so we could move my mom there from Colorado Springs. Although opening a new place always begins with challenges, the staff and volunteers of Forest Ridge have been amazing! Dealing with memory care is such a challenge for the agency as well as the family and everyone involved. There has been a marked improvement in my mom since she moved in and I continue to marvel at the great loving care she receives. Thank you so much for giving my mother back her dignity! This place doesn't even deserve 1 star. Please do not place your beloved family member here. I have painfully regretted it since shortly after my mom went there.
Beautiful location and facility. Feels like home to our residents. Great place to work with caring staff and great teamwork. Patients are treated with utmost dignity and respect.

At first when my mom was placed here I believe it was in 2021, the staff was great and my mom was treated the way she should have....with respect. Now, all the nice people are gone, my mom isnt getting the right treatment for her health problems and shes lost a lot of weight. They should be ashamed of themselves and that place needs to be investigated big time. I gotta get my mom out, fast!! Its just sad because its such a beautiful place in a beautiful setting but that aint nothing when your mom is getting to the point she doesn't want to live anymore.
