In support of Allen County and its municipalities, the Fort Wayne-Allen County Department of Health strives to promote the health, safety, and well-being of all residents. With guidance from the Indiana State Department of Health and support from our many partnering agencies, we endeavor to have a model public health infrastructure that provides professional, responsive, and cost-effective services to the community.

An easy, swift experience getting a certified copy of my birth certificate. I’ve been stressed about getting my passport in time and finally got a same-day appointment in Chicago. Because a different agency is holding my birth certificate, I had nothing to show. It took 10 mins and I was out the door. Thank you for your help in my time of stress! :

Wow I couldn't believe how smoothly my visit went. I wanted to get some free at home COVID testing kits. The lady at the desk handed them to me immediately. Didn't press me on why I wanted to get tested, if I had symptoms, didn't make me fill out any paperwork, etc. I was in and out in less than 10 minutes with my testing kits. Thank you for such a simple and prompt visit!

Somehow...this group of experts are now running our lives. Limiting only 250 people for HS football games....Limiting only 65 players to dress...Cowards.  You people have no clue about the real world.  You have no idea about how children learn and mature.  Covid-19 isn't the problem....you are!!!!!

They provided me with great service. I was in and out. Everyone I encountered was plea and professional.

People can't see their Doctor unless you force people to wear mask.

Can you get a copy of your birth certificate here ?? i lost mine and have been looking up how too get it
