Heritage Park Nursing Center focuses primarily on post-hospitalization transitional care. Our greatest success is the ability to help patients recover and discharge back to home or to a lower level of care! We employ our own therapists and operate a large, fully contained therapy unit. Together, with our nursing staff, our goal is to help patients achieve their highest level of functional independence.

Our journey began when our dad was placed in a horrible nursing home directly from the VA Loma Linda Hospital with no warning.  One Flew Over the Cookoo's nest should explain it! After some research I got hold of Carmen in admissions at Heritage Park in Upland, she was absolutely fantastic and worked hard to get him transferred out of Riverside and closer to us. We have been very please with everyone at Heritage and the around the clock care is fantastic.   Each time we have been there we are greeted with smiles from everyone,  the place smells clean and is communication with the RN's, LVN's and CNA's is a true blessing.   We felt lucky to have our loved one so close and well taken care of. To top it off our greatest non medical help is with the social workers office.  Sandra is not only knowledgeable as to what we needed but took the time to help us with the transfer our dad out and to an assisted living facility.  In addition Mike, the administrator, listen any time we had a need and was on top of it! I could go on about the physical and occupational therapy, the kitchen staff, the custodial and the front desk because they hold there own too. Longest review I've ever written but trust me when I say it is well deserved!!
My mom had been at Heritage Park since August 2020 when we can't even visit her in her room because of COVID. The staff was accommodating so we could talk to each other and putting her by the exit door with a cell phone that she could use. She always looks well groomed. The nurse calls me with health updates and to my surprise, a house dentist called me with a dental checkup report. Currently, I am now able to visit mom in the TV room and touch her. This is the first time I'm able to go inside the facility. It is clean and doesn't smell like the other nursing home facilities that I've been in the past. The nurses, receptionist and the admissions staff Jeremiah are friendly and very helpful. I know they give mom the best care that she needs. I would highly recommend this facility who wanted to put their loved ones here cause I know they will get an excellent care from the staff.
9 months and counting. My mother has been at this place for nine months now. I continually write reviews because I want others to know that this skilled nursing facility is one that sets the pace and standards that others should notice. Micayla the Activity director and her staff makes the patients feel that they are an extended family member. From the dining area with table class to scheduled activities during the day and let’s add there once a Month outings . It is nice to know that your love one is not going to be bored. The staff at Heritage have a great way of including staff and patient interaction. I definitely recommend this place. Sharon
Take my advice, do what ever you can and don't send your loved one to Heritage. Even if San Antonio or another hospital recommends them. I am not making this up and I am not paid to write this review. They neglect their patients, their facility had a Covid breakout, and they with the insurance companies do what ever they can to hurry the process to get your loved one transferred out. I can go into more detail, but to save you the time, just don't send your loved one here. It will be one of the worst decisions you make. Update: And of course they reply to this review with an automated message that is sent after reviews. As evidence they don't care. They get their fair share of patients that have no idea the problems this facility has till its too late
My mom has brain cancer and was admitted to heritage, at 1st they were great but as my mom's condition started declining they got really frustrated with her and mistreated her they practically forced me to take her out and transfer her to another facility. When I picked her up they threw all of her items in bags and her suitcase not organized and she had on her bathrobe with a diaper and tank top they didn't even bother to dress her. unless your loved one does not have significat conditions do not admit there they don't like to hassle with challenging patients all they think about is their numbers. Also social worker Sandra is very rude and not professional. My mother was transferred about a month ago and till this day she has not called to have a decent conversation about her discharge, the way they handled her discharge was so unprofessional. I'm glad to say that I have found a better place where they really do care for their patients.
The facility is OK and the care provided is adequate. But the staff, while pleasant could use an infusion of inspiration to truly care for the patients there. They frequently are delayed in delivering medication you will find 6 of them standing around talking and there is a brush off culture there. When you speak with them about your concerns or the needs of your loved one, they are all too quick to pacify you with some response and then they move on with the issue unresolved. They are highly disorganized. My impression is the staff, just isn't anchored in to what they are doing and the very important job they are doing. They operate on the surface, with no true intention of creating a best quality of life possible for these poor people who have lost their ability to function in normal life. It seems to be a problem in leadership. I feel it could greatly improve with a deeper ingrained philosophy, training and proper supervision.
Had taken a relative there for physical rehabilitation, since we lived close at the time. They were robbed of their dignity, wouldn't get help when they needed it, and was left to urinated on themselves in their bed, among several other things. Worst experience. If I had money at the time I would have pursued legal action against them. Stay away from this place.
I highly recommend this rehab facility. It is the nicest one around. Jeremiah Johnson is kind and helpful. My husband was a patient here. The facility is clean and inviting. The staff is patient and caring. An added plus PT and OT are on the premises! There is a pretty court yard and common areas where family and friends can visit .
This place is very nice. I had a family member there and she really liked it and the staff. She always stated the nurses and caregivers were always helpful and very caring. I myself was impressed with the layout and cleanliness of the faculty. Administration and staff were very helpful in my family’s time of need. Thank you so much for taking good care of my sister
My FIL is a new pt at your facility. He’s a Diabetic and recovering from Quadruple Bypass Surgery. His blood sugar was 300 today, because he has not been given his insulin for the past two days. This is unacceptable! His nurse this morning told him his chart says he’s diabetic, but doesn’t say he needs insulin. I’ve just emailed your facility. If anything happens to my FIL because he did not receive his insulin, your facility will be held accountable.
