Home Care, Advance Illness Management, Supportive Services, Private Duty Services, Hospice Care and Community Hospice House. Available services include: nursing, IV, psychiatric, maternal/child health, pediatric and wound care. Hospice care provided in the home and nursing home. HHHC proudly owns and operates the Community Hospice House in Merrimack, NH. Physician and Advanced Practice Nursing Services, hospice nursing, pain and symptom management, bereavement care and counseling, spiritual care, medical social work are available in hospice services. Rehabilitation services offered include physical, occupational and speech therapy. Rehab nursing and pre-operative home safety program also offered. Our private duty division has licensed nursing assistants, homemakers and companions that can assist with light meal preparation, transportation to medical appointments, picking up prescriptions, grocery shopping and light housekeeping, Alzheimer's respite and other services. Community Services available include blood pressure clinics, foot care clinics and free grief support groups for adults and children.
