Live Healthy Lincoln County Coalition is working on community prevention strategies focused particularly on those at high risk, to prevent diabetes,heart disease, and stroke. Health system interventions and efforts to link community programs to clinical services is the key goal. This effort is made possible thru a grant from KDHE and CDC.
