The P.A.C.T. Team is a client-centered, recovery-oriented mental health delivery model that emphasizes support for community living, psychosocial rehabilitation, and recovery for individuals with the most severe disabling mental illnesses.
The P.A.C.T. Team is generally described as a "hospital without walls" or a "mobile hospital." P.A.C.T. stands for Program for Assertive Community Treatment. The treatment team approach is to provide effective treatment, rehabilitation, and support services to clients with the overall goal of improving the client's quality of life and reducing the number of hospitalizations. The P.A.C.T. model provides a variety of services including: service coordination, crisis assessment and intervention, symptom assessment and management, individual counseling and psychotherapy, medication prescription, medication administration and monitoring, substance abuse treatment, work-related services, activities of daily living services, social and interpersonal relationships, leisure-time activity services, direct assistance to ensure clients obtain basic life necessities, and education and consultation to clients' families and other major supports. The P.A.C.T. Team believes in empowering clients to learn the skills they need to make healthy choices, and work on establishing and maintaining stability. The P.A.C.T. Team provides at least three contacts per week, which allows team members to work with clients towards effective linkage, advocacy, and referral services. The P.A.C.T. also provides a variety of P.A.C.T. groups/functions including: addictions group, peer support group, shopping group, bowling group, movie group, walking group, health and wellness group, community exploration group, fishing trips, picnics, barbecues, holiday events, and other seasonal groups/functions. The P.A.C.T. Team strongly works towards the goal of the Mental Health Center in "Offering Hope-Sharing Solutions."
