Trusting a facility with a loved one who is vulnerable has to be one of the hardest decisions we may ever be faced with but the staff and administration at Park Manor have made it so much easier! From the start of admissions when Eileen showed integrity, compassion, and efficiency I knew this was the right decision for us. So many horror stories in this field that everyone should know THIS FACILITY IS THE DIFFERENCE AND GREATLY RECOMMENDED! NO WHERE COMPARES WHEN IT COMES TO THE NURSES AND ADMINISTRATION! To have my mother actually request going here after having a short stay for rehab helps to ease your worries. The nurses, therapists, case workers, admissions, and support staff of many kinds are all there with love and sincerity for their patients family continuously going the extra step. They truly want the best for those they are taking care of. I hope you all know how much you mean to the families that you're supporting! I had to leave this review because you simply deserve it.

From the time of my initial conversation with Darla in admissions, I felt welcomed and valued as a family member whose husband needed lots medical care after surgery. I was asked about goals, care plans, and expectations. The social workers put alot of effort into care plans before and after a patient's stay. The nurses are informative, beyond caring, and helpful above the call of duty! Thank you Jennifer, Angie, Edgar and everyone!!! The desk staff at the facility greets you warmly. The follow-up care team calls and checks in about our progress and needs. Overall it's clean. Facing the challenges of today's medical community, even when short staffed, they aced their care, compassion, and showed lots of support to patients AND their families!

My mom is a resident of the facility since October 2019. The care, compassion and communication from the staff and team members is outstanding. I reside out of state so it is comforting to know she is in good hands.

My mother in the Memory Unit 601 passed away this week. While her stay of a few months was brief the care she received was outstanding. The staff was wonderful. From Reception, all the Nurses and CNA's in the unit , the lovely ladies who took care of her laundry, Supervisors and the entire care team. Deciding to have a love one placed in a long-term care facility is never an easy decision, but bringing her here was a blessing. I wholeheartedly thank you, I will miss seeing everyone.

My three siblings and I are very happy with the care our mom received at Middletown Park Manor. Over the course of the 14 months our mother resided at Park Manor, the Nurse Manager continuously gave us updates on changes to her medical status; the communication was excellent. As a result of dementia, Mom could be tough and unreceptive to ADL cares. Despite that, the caregivers' exhibitied professionalism, compassion and kindness. During the pandemic the Activities team always made time for the family with facetime or zoom calls, so we could speak to our mom and see her. When Mom's health declined, she received excellent end of life care. The Nurses, Certified Nursing Assistants, Activities staff, Director of Nursing, Speech Therapist, Nursing Supervisors, and Housekeepers stopped in to check on her, and this occurred over all three shifts. They also made sure the family members at bedside had food, beverages, and answered any questions we had. The staff were genuinely affectionate toward mom. We are truly grateful to everyone at Park Manor, as her final days were comfortable and peaceful.

My Mom is a resident at Park Manor and is 97 years old. The time during the pandemic due to the lack of her being able to have visitors, was extremely difficult for her. The staff who care for her became her family and they treated her like family. Staff, like Tara G. made it possible for myself and my siblings, to Face Time with my Mom. There are 7 of us, and 6/7 were able to Face Time with my Mom, two of us per week. It gave us such comfort to be able to lay eyes on our Mom and chat with her. At times the facilitator had to also relay what we were saying because my Mom couldn’t hear us very well. Tara G. and Nicole from the Rec Dept never hesitated to do that for us. All the staff are to be commended for the superb job they did during that horrific time. From the kitchen staff, to the cleaning and laundry people, OTs, PTs, Rec Dept, activities Director, and of course Laura C on Unit 2, and Nicole, the Social Worker who has been more than accommodating setting up appointments for us to see our Mom now that visitation is allowed. My siblings and I can never be able to repay you for taking such wonderful care of our Mom. Thank you doesn’t begin to describe the gratitude we feel towards them all.

When my grandmother fell and needed attentive care the team at park manor gave exceptional service both medically and emotionally! A woman her age could feel out of place and scared with a break/fracture that limited her mobility but the care she was given made her feel at ease, comfortable and safe; she even made a lot of friends in a short stay. Thank you again to the team for always going above and beyond to take care of our loved ones in ways that only you can provide!

Park Manor Rehab is staffed by dedicated physical and occupational professionals. The nursing staff is excellent. They care about their patients. My sister was very ill and had a broken arm when she came to Middletown Park Rehab, she is now thriving and improving every day. Thank you to the staff and administrators for striving to be the best.

Tara G from the rehab department works on all units is an outstanding employee and even better person. If you're under her care, you know that your comfort and needs are priority. Tara works many extra hours tirelessly and still does so with a smile on her face!!
