The OSU Health Access Network provides case management and coordinated care through referrals and data analysis to provide and improve access to specialty care for OSU SoonerCare Choice patients.
Our team of Nurse Case Managers work very closely with the clinic's physicians and clinical staff to ensure quality of care, while also providing assistance to SoonerCare Choice patients. Our Nurse Case Managers can assist patients with transportation needs, medication management, clinic appointment logistics, health education, social resources, and much more.
