2. Reduce patient health care costs
On Point Primary Care wishes to separate primary care from the traditional health care plans. Primary care consists about 90% of the majority of health care needs; from the acute needs to the management of chronic issues. Focus here is on a more economical manner will allow “insurance” to be used as it was intended for the rare and expensive events. By humanizing the physician-patient experience we can help prevent the need of costly insurance and prevent costly events by focusing on health not just disease. We can be preventive.
3. Change health care incentives
On Point Primary Care would like to replace the traditional fee-for-service (FFS) incentives of volume with value. On Point Primary Care truly is a patient centered home. The patient-physician relationship is the foundation of this model. No longer does the physician get paid based on the number he/she sees. The rewards are based on the quality of medical service that he/she provides. Replacing FFS with a simple monthly subscription that cover comprehensive primary care services helps achieve this. Primary healthcare then is carved out of the “insurance” concept with the removal of the overhead of that system then primary care becomes affordable again. Attention is given to appropriate size patient panels to support this level of care and commitment.
4. Improve Patient Experiences
On Point Primary Care was created for patients, designed to incentivize quality rather than quantity for less cost. In addition to medical services found in the primary care setting, patients typically receive greater attention through longer visits, 24/7 access to the physician, less wait times, same day or next day appointments, secured telemedicine communications, and emphasis on prevention of ill health. A new Direct Primary Care practice different from the typical primary care practice. For example:
• Patients will pay for a broad range of specified primary care services with regular (e.g., monthly) subscription fees.
• Physicians and other members of the care team will spend more time with the patient during the office visit compared with a typical primary care visit. ; 30-60 min visits vs 10-15.
• The practice will focus on building strong patient-physician relationships.
• The practice will not file claims with third-party payers/insurance companies, which will eliminate certain staffing costs.
In addition to covering the cost of office visits, in-office labs, and office procedures, this monthly fee will give my patients:
• Access. They can access me via phone, secure email, or text messaging. Since I no longer have to see them to be paid, I can handle many of their problems without requiring them to leave work or home.
• Enhanced access to their medical records. I will work with them to build and maintain an accurate health summary. My hope is that this will not only help them, but also better inform any other physicians involved in their care.
• My attention, care and advocacy. I can finally make sure that my patients will be up to date on the care they should be getting. I can also take the time to coordinate the care they get from me, from specialists, or at the hospital.
• Better information and better care.
